General Information about Southern Orthopaedics and Sports Medicine
Office hours are 8:00 A.M. until 5:00 P.M. Monday through Thursday and Friday from 8:00 A.M. until 12:00 P.M. There is a doctor on call 24 hours a day. Emergencies are treated at the Baptist Hospital Trauma Center.
For your appointment please call (850) 432-3293 as soon as possible to assure proper follow-up care. If you find it necessary to cancel your appointment, please give our office 24 hour notice.
NO SHOW POLICY: Failure to cancel appointment prior to 24 hours of the scheduled appointment will result in a $25.00 "No Show" fee.
We are providers for numerous insurance carriers. You may contact our office to verify participation in your particular plan. Please bring your insurance information and coverage identification cards at the time of your appointment. We ask that you pay for your deductible, co payment, and uncovered medical services and supplies at time of service. Individuals with no insurance coverage are asked to pay for all office visits, x-rays and medical supplies at the time these services are rendered. Although we do file insurance for some of our services, we are unable to file for balances less than $25.00 (primary or secondary). Additionally, if your insurance company refuses payment for any reason, we must look to you for payment of your account. If you have any questions, our insurance department will be happy to assist you. Please call 432-3293.
We have instructed our assistants to handle all incoming telephone traffic. This enables the physician to attend to the needs of our patients with a minimum of interruptions. Please be patient. Due to our specialty, we spend a large portion of our time in the operating room; therefore, there will be times when we cannot be back with you as quickly as we would like. Please be assured that your call will be handled as promptly as possible.
Since it may take some time to contact your physician, we request that you call our office (3) days in advance for all prescription refills. In order to avoid any errors or complications, please have your pharmacy name and phone number available at the time of your request. Please request refills prior to Friday afternoon.
Our fees vary according to the complexity of your problem and the place where care is rendered (hospital, office, emergency room, etc…). We will be happy to discuss our fee schedule with you at any time. Please keep in mind however, that filing insurance claims is an additional service we provide. And, although we will be glad to assist you with collecting from your insurance company, we can not accept responsibility for collecting your insurance claim or for negotiating a settlement of a disputed claim.
RELEASE OF INFORMATION
Report concerning your health will not be released to any third party without your written authorization. Due to legal reasons, x-rays are the property of our practice. If you would like copies there will be a charge for reproduction.
LIABILITY & WORKER’S COMPENSATION
Payment of the bill is the responsibility of the individual who has received treatment, regardless of the circumstances surrounding the injury.
In all workers compensation cases, an authorization for treatment must be obtained prior to your visit. Any assistance you can provide may reduce your appointment waiting time.
